Telephone first System
Tyn Y Coed Surgery will employ a Doctor led Triage System. Patients/carers who telephone or attend the surgery requesting appointment are advised by the receptionist that a member of the team will telephone the patient or carer back either as a telephone consultation or to arrange a face to face appointment. To ensure this system works best receptionists will need to have upto date contact details and will ask for a brief indication of the problem/symptoms. The receptionists will ask for this information so that calls can be prioritised by the triage team accordingly. Receptionists can also help signpost patients to the most relevant service. All receptionists operate to the highest standards of confidentiality within the NHS and under the GDPR regulations.
The practice will endeavour to respond to requests for an appointment on the day the request is made if the request comes into the practice by 11.30am
If the matter is urgent the receptionist will alert the triage team or on-call doctor and the patient concerned will take priority. If it is a life threatening emergency please call the emergency services by telephoning '999'.
Requests for an appointment are likely to result in the following outcomes
· Telephone Advice (Delivered by receptionist after advice from the Doctor)
· An appointment with the Doctor
· A telephone consultation with the Doctor
· A request to submit an e-consult
· An appointment with our Nurse Practitioner/Practice Nurse/ HCA
· Home visit (for palliative ,housebound patients only)
We hope by developing this system patients will get access to the GP team more quickly. To maximise this however we would expect that any requests for routine appointments would be processed on the same day if a request is received first thing in the morning. After this time routine requests will be asked to use eConsult or to phone back the next day or the day on which the appointment is needed
We will be reviewing the new system as it develops and would be great full for any feedback
We anticipate patients will make appointments either by telephone or online via MHOL but appreciate that there will be rare circumstances whereby patients may need to attend surgery to make their appointment.
